How do I change my name on my previously issued award certificate?

After conferral, changes to your name cannot be processed automatically and must follow a defined two‑step process.
Step 1: Change of Personal Details (mandatory)
You must first complete the Change of Personal Details Form and submit it to Curtin Connect for processing.

- You should contact Curtin Connect if you require assistance completing or submitting the form.
If this step has not been completed, the request cannot proceed.
Step 2: Replacement award application
Once the Change of Personal Details has been processed, the student must then:
- Complete and submit the Replacement Award online application form, and
- Pay the applicable fee to amend the formal name and reissue the award with the revised name.
Return of original award
If an original award certificate has already been issued, it must be returned to Curtin University as part of the replacement award process.
Replacement awards will not be issued until:
- the fee has been paid, and
- the original award certificate has been returned (where applicable).
