How do I change my name on my previously issued award certificate?

You will need to order a replacement award by completing an online application. A fee will be payable to amend the formal name and reissue the award with the revised name. The original award certificate must be returned to the University.
Please note you will need to attach photo identification documents (e.g., drivers license, passport or Student ID card) in compliance with Curtin University's Document certification requirements.
