The University continues to evolve its maturity in managing our privacy obligations at Curtin in order to be ready for compliance with the Privacy and Responsible Information Sharing Act (WA) 2024 when it takes full effect in July.
As a part of these changes, we have launched a revised privacy reporting process, which is easy to use and readily available to all staff and students.
You can find the privacy incident reporting form in ServiceNow.
What is the form for?
This form is for reporting any privacy incident, concerns about your privacy, allegations related to a breach of privacy or accidental disclosure of personal information to the wrong person.
It will also be used for the reporting of all Information Breach incidents under the Information Breach Policy and Procedures.
Anonymous reporting
Anonymous and proxy incidents, as well as incidents lodged by parties external to Curtin, should be reported directly to the privacy officer via email. If the complainant wishes to remain anonymous, their details will not be added to the incident ticket created by the privacy officer in proxy.
Privacy will continue to work alongside all stakeholders across the organisation to streamline reporting and delegation of incidents as required for reporting and compliance purposes, including matters where conduct may be investigated by the Academic Registrar or Employee Relations.
As a part of this change, Incidents reported to the Integrity and Standards Unit as a Breach of Privacy will all now be administratively moved to the new form and process via ServiceNow.
