When you are booked as a new patient, you will receive a text message from Hotdoc which includes a link to an online registration form. <\/p>\n\n\n\n<p>If you did not receive the link, please use the link below.<\/p>\n\n\n<mimas-block \n        block-name=\"button\"\n        \n        data-align=\"left\"\n        role=\"complementary\"\n        aria-label=\"Button block\"\n        data-segment=\"all\">\n\n    \n    \n<div class=\"block-heading hidden\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"button-block\">Button block<\/h2>\n            \n                        \n                        \n            \n                <\/div>\n\n                    <a\n    href=\"https:\/\/www.hotdoc.com.au\/forms\/curtin-university-patient-registration-form-1\"\n    class=\"button  button--deep-blue filled\"\n    target=\"_blank\" aria-label=\"Open the Patient Registration Form\">Open the Patient Registration Form<\/a>\n    <\/mimas-block>\n\n\n<p>If you have an overseas health insurance policy, you must include details of the policy\/membership number. <\/p>\n\n\n\n<p>For all patients who do not have a Medicare card, we encourage that you obtain an Individual Healthcare Identifier (IHI) which allows you to obtain prescriptions electronically (via tex) and helps you keep your prescriptions safe and secure. This function is not available if you do not have an IHI number.<\/p>\n\n\n\n<p>For more information on Individual Healthcare Identifiers, visit the <a href=\"https:\/\/www.servicesaustralia.gov.au\/individual-healthcare-identifiers\" target=\"_blank\" rel=\"noreferrer noopener\">Services Australia website<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Cancelling an appointment\/Non-attendance<\/h2>\n\n\n\n<p>We require a minimum of 30 minutes notice to cancel your appointment. This will allow us to offer your appointment to another patient who may need to see a doctor.<\/p>\n\n\n\n<p>Failure to cancel your appointment within the appropriate timeframe may result in a fee between $40-$60. This fee will need to be paid before you can make your next appointment.<\/p>\n\n\n\n<p>Patients who have provided us with their mobile numbers will receive a text message from Hotdoc asking them to confirm or cancel their appointment. If your test result requires urgent attention, our Practice Nurse will contact you with the necessary details.<\/p>\n<p>For non-urgent results, a text message will be sent to you asking to make an appointment with the GP.
