This includes requesting changes to your ethics approval, and letting us know how you are progressing. All submissions are to be made via <a href=\"http:\/\/infoed.curtin.edu.au\">InfoEd<\/a>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"#annual\">Annual reports<\/a><\/li>\n\n\n\n<li><a href=\"#completions\">Completions<\/a><\/li>\n\n\n\n<li><a href=\"#change-investigator\">Change of investigator<\/a><\/li>\n\n\n\n<li><a href=\"#amendments\" data-type=\"internal\" data-id=\"#amendments\">Amendments<\/a><\/li>\n\n\n\n<li><a href=\"#reportable\">Reportable events<\/a><\/li>\n<\/ul>\n\n\n<mimas-block id=\"annual\"\n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n<div class=\"block-heading\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"annual-reports\">Annual reports<\/h2>\n            \n                        \n                                <div class=\"block-heading__content\"><p>Ethics applications are initially approved for a 12-month period, which can be extended each year upon submission of an Annual Report. You will receive automated reminder emails at 8 weeks before the expiry date, and again at 4 weeks before the expiry date.<\/p>\n<p>If your research is still ongoing, submitting an Annual Report will extend the approval for another year, however if the research has finished, has been discontinued, you can submit a Completion Report.<\/p>\n<p>If you are finished with recruitment and data collection, but are finalising a thesis or manuscript, you can close the record via a Completion Report. But, if there is any chance you will need to collect more data, then please submit an annual report to keep the record open, just in case.<\/p>\n<\/div>\n            \n            \n                <\/div>\n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-Things-to-note---Reciprocals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"Things-to-note---Reciprocals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-Things-to-note---Reciprocals\">Things to note &#8211; Reciprocals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"Things-to-note---Reciprocals\" aria-hidden=\"true\">\n                    <p>If you have a reciprocal approval at Curtin, with another institution as the lead HREC, you will only need to submit an annual report when InfoEd prompts you to &#8211; that is, when the expiry date is coming up. We reciprocate the end date that the lead HREC have given, and we do not require you to complete annual reports every 12 months. If the end date is 5 years away, InfoEd won&#8217;t prompt you for an Annual Report for 5 years!<\/p>\n<p>However, if the end date of the approval does approach and you are prompted to submit an annual report to extend the research, it is <strong>essential that you provide a new letter from the lead HREC<\/strong> with the new end date of the ethics approval. We cannot proceed without this evidence. <\/p>\n\n        \n                            \n                    <\/div>\n<\/div>\n        \n<\/mimas-block>\n\n<mimas-block id=\"completions\"\n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n<div class=\"block-heading\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"completions\">Completions<\/h2>\n            \n                        \n                                <div class=\"block-heading__content\"><p class=\"PDq2pG_selectionAnchorContainer\" data-start=\"2913\" data-end=\"3107\">A completion report must be submitted when all research activities have concluded. This enables the ethics office to formally close the ethics approval and maintain accurate records of approved research.<\/p>\n<p data-start=\"3109\" data-end=\"3157\">A project is generally considered complete once:<\/p>\n<ul data-start=\"3158\" data-end=\"3303\">\n<li data-section-id=\"1wg79sg\" data-start=\"3158\" data-end=\"3193\">participant involvement has ended<\/li>\n<li data-section-id=\"y9lppi\" data-start=\"3194\" data-end=\"3224\">data collection has finished<\/li>\n<li data-section-id=\"130rsn6\" data-start=\"3225\" data-end=\"3303\">there is no further interaction with participants requiring ethics approval.<\/li>\n<\/ul>\n<p data-start=\"3305\" data-end=\"3437\">Researchers should ensure all outstanding reportable events and amendments have been resolved before submitting a completion report.<\/p>\n<\/div>\n            \n            \n                <\/div>\n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-Things-to-note---Reciprocals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"Things-to-note---Reciprocals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-Things-to-note---Reciprocals\">Things to note &#8211; Reciprocals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"Things-to-note---Reciprocals\" aria-hidden=\"true\">\n                    <p>We do <strong>not<\/strong> need evidence from the lead HREC in order for you to submit a completion through InfoEd.<\/p>\n\n        \n                            \n                    <\/div>\n<\/div>\n        \n<\/mimas-block>\n\n<mimas-block id=\"change-investigator\"\n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n<div class=\"block-heading\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"change-of-investigator\">Change of investigator<\/h2>\n            \n                        \n                                <div class=\"block-heading__content\"><p>When there are any changes to your research team, submit a change of investigator through InfoEd. Use the same form for changing the CI, removing investigators, adding investigators.<\/p>\n<p>When adding new investigators, please also attach a copy of their Research Integrity Training (RIT) certificate<\/p>\n<\/div>\n            \n            \n                <\/div>\n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-Things-to-note---Reciprocals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"Things-to-note---Reciprocals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-Things-to-note---Reciprocals\">Things to note &#8211; Reciprocals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"Things-to-note---Reciprocals\" aria-hidden=\"true\">\n                    <p>If you are wanting to add non-Curtin researchers to the ethics record, you must attach evidence from the lead HREC showing they have been approved.<\/p>\n\n        \n                            \n                    <\/div>\n<\/div>\n        \n<\/mimas-block>\n\n<mimas-block id=\"amendment\"\n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n<div class=\"block-heading\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"amendments\">Amendments<\/h2>\n            \n                        \n                                <div class=\"block-heading__content\"><p class=\"PDq2pG_selectionAnchorContainer\" data-start=\"942\" data-end=\"1134\">If you need to make changes to an approved research project, you must obtain approval before implementing the changes, unless they are necessary to eliminate an immediate risk to participants.<\/p>\n<p data-start=\"1136\" data-end=\"1170\">Amendments may include changes to:<\/p>\n<ul data-start=\"1171\" data-end=\"1448\">\n<li data-section-id=\"gz1yni\" data-start=\"1171\" data-end=\"1209\">the research protocol or methodology<\/li>\n<li data-section-id=\"qzqkrs\" data-start=\"1210\" data-end=\"1259\">participant recruitment or eligibility criteria<\/li>\n<li data-section-id=\"34axw1\" data-start=\"1260\" data-end=\"1309\">participant information sheets or consent forms<\/li>\n<li data-section-id=\"lty5qa\" data-start=\"1310\" data-end=\"1369\">questionnaires, interview guides or other study documents<\/li>\n<li data-section-id=\"tw74p5\" data-start=\"1370\" data-end=\"1405\">research personnel or study sites<\/li>\n<li data-section-id=\"12mlf9k\" data-start=\"1406\" data-end=\"1448\">study timelines or funding arrangements.<\/li>\n<\/ul>\n<p data-start=\"1450\" data-end=\"1640\">Submit an amendment request with sufficient detail to enable the HREC to assess the proposed changes and their potential impact on participants and the ethical acceptability of the research.<\/p>\n<\/div>\n            \n            \n                <\/div>\n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-Things-to-note---Reciprocals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"Things-to-note---Reciprocals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-Things-to-note---Reciprocals\">Things to note &#8211; Reciprocals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"Things-to-note---Reciprocals\" aria-hidden=\"true\">\n                    <p>If Curtin has reciprocated the ethics approval of another HREC, we will need to also reciprocate any amendments that are approved throughout the course of the ethics approval. You will need to submit the approval letter and supporting documentation through InfoEd for the Curtin Ethics office to review as soon as the lead HREC has approved the amendment, and before the changes are implemented.<\/p>\n\n        \n                            \n                    <\/div>\n<\/div>\n        \n<\/mimas-block>\n\n<mimas-block id=\"reportable\"\n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n<div class=\"block-heading\"\n     data-segment=\"dom\"\n     data-style=\"\"\n     data-align=\"default\"\n>\n\n                                <h2 class=\"block-heading__title \" id=\"reportable-events\">Reportable events <\/h2>\n            \n                        \n                                <div class=\"block-heading__content\"><p class=\"PDq2pG_selectionAnchorContainer\" data-start=\"2156\" data-end=\"2331\">Researchers must promptly notify the HREC of any reportable events that may affect the safety, welfare or rights of participants, or the ethical acceptability of the research.<\/p>\n<p data-start=\"2333\" data-end=\"2363\">Reportable events may include:<\/p>\n<ul data-start=\"2364\" data-end=\"2736\">\n<li data-section-id=\"1nqjv0h\" data-start=\"2364\" data-end=\"2388\">serious adverse events<\/li>\n<li data-section-id=\"tg35xd\" data-start=\"2389\" data-end=\"2424\">protocol deviations or violations<\/li>\n<li data-section-id=\"1lckwf1\" data-start=\"2425\" data-end=\"2544\">unanticipated problems or new information that may affect participant safety or willingness to continue participation<\/li>\n<li data-section-id=\"g9ewlx\" data-start=\"2545\" data-end=\"2597\">complaints relating to the conduct of the research<\/li>\n<li data-section-id=\"1rjezjc\" data-start=\"2598\" data-end=\"2650\">significant breaches of privacy or confidentiality<\/li>\n<li data-section-id=\"zakytq\" data-start=\"2651\" data-end=\"2736\">suspension or termination of the project by another institution or regulatory body.<\/li>\n<\/ul>\n<p data-start=\"2738\" data-end=\"2887\">Prompt reporting enables the HREC to assess whether any action is required to protect participants or maintain the ethical integrity of the research.<\/p>\n<\/div>\n            \n            \n                <\/div>\n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-Things-to-note---reciprocals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"Things-to-note---reciprocals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-Things-to-note---reciprocals\">Things to note &#8211; reciprocals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"Things-to-note---reciprocals\" aria-hidden=\"true\">\n                    <p>If a reportable event occurs in research that has been reciprocally approved, you will need to notify the lead HREC first, but we also need to be made aware.
