Not long now.<\/span><\/p>\n<p><span class=\"font-arial\">Following on from the important information you received regarding\u00a0<a href=\"https:\/\/covid-19.createsend1.com\/t\/d-i-autiult-l-y\/\" target=\"_blank\" rel=\"noopener\">vaccination mandates<\/a>\u00a0for staff and students, you are required to provide evidence of your vaccination status (or exemption) to the University.<\/span><\/p>\n<p><span class=\"font-arial\">All students accessing a Western Australian Curtin University campus or venue or participating in an \u201cin-person\u201d activity organised by Curtin, must comply with the vaccination mandate. <\/span><\/p>\n<p><span class=\"font-arial\">Please complete the <a href=\"https:\/\/curtin.t1cloud.com\/T1Default\/CiAnywhere\/Web\/CURTIN\/Forms\/FillOut?formDef=ba17dd5a514cc6f6989fb3a88b078c4f&#038;suite=CES\">COVID-19 Vaccination Status Collection webform<\/a>.<\/span><\/p>\n<p><span class=\"font-arial\">This link can also be accessed via OASIS and eStudent.<\/span><\/p>\n<p><span class=\"font-arial\">If you aren\u2019t already logged in to OASIS you will be prompted to do so before you can access the form.<\/span><\/p>\n<p><span class=\"font-arial\">Ensure you have your vaccination dates and\/or evidence ready to upload to the form. Your vaccination certificate must contain all COVID-19 vaccinations you have received, and for this reason screenshots of the ServiceWA app certificate will not be accepted. For more information on acceptable vaccination documentation, please refer to the\u00a0<a href=\"https:\/\/covid-19.createsend1.com\/t\/d-i-autiult-l-t\/\" target=\"_blank\" rel=\"noopener\">Student Vaccination FAQs<\/a>.<\/span><\/p>\n<p><span class=\"font-arial\"><strong>NOTE:<\/strong>\u00a0Students must redact their IHI number from their vaccination certificate before uploading into OASIS.
